Parents must complete the required registration forms prior to the first day of school.

• Authorization for Emergency Medical
• Authorized Pick Up Form
• Physician’s Medical Form
• Copy of Immunization Records
• Enrollment Contract

In accordance with New Jersey law, health and age requirements must be met for children to enter preschool. A complete health check-up and all required immunizations must have been received prior to admission. A report of the health examine, which include an immunization record, must be properly filled out and signed be a licensed physician.
